Responsibilities:
- Formulates client care plan for optimum outcome.
- Performs continuous assessments and evaluations to ensure patient is progressing toward desired outcomes.
- Works collaboratively and maintains active communication with physicians, providers and multi- disciplinary treatment team.
- Proactively addresses/resolves barriers that hinder effective client care using advanced conflict resolution skills.
- Monitors the patient’s progress, intervening to ensure that the plan of client care is focused and effective, modifying as necessary, to meet the ongoing needs of the client.
- Communicates to third-party payers maintaining continuity of care for client.
- Maintains knowledge of resources and facilities available to patients and family members.
Qualifications:
- Bachelor’s degree required
- BSN preferred
- Current unrestricted Michigan registered nursing or occupational therapy license
- 5-7+ years of experience, preferred
- ICU, ER and/or home health experience, a plus
- On the job training available
Skills:
- Excellent critical thinking, communication and interpersonal skills
- Ability to work independently and exercise sound judgment
- Strong organizational and time management skills
- Proficient in MS Word, Outlook and Excel

Requirements:
- Five years’ office experience, preferred
- Excellent writing, editing and proofreading skills
- Proficient working knowledge of Microsoft Word, Excel and Outlook
- Extremely detail oriented with a high degree of accuracy
- Exceptional communication skills, both verbal and written
- Ability to meet deadlines successfully
- Ability to follow instructions and work independently
- Strong team skills
- Some knowledge of medical terminology is helpful
- Previous work experience in a healthcare setting is a plus
- Mature disposition and positive outlook
